Peritoneal Dialysis
Our peritoneal dialysis offers flexible and individualized care for patients requiring management of their kidney disease. Supported by a team of highly-trained nurses and CNAs who undergo comprehensive training and competency programs – our programming maximizes the progress of each patient’s treatment.
Wound Care
We utilize an innovative, interdisciplinary approach to treat chronic wounds. This multi-system approach relies on advanced treatments and therapies, including E-Stim, ultrasound, negative pressure therapies, ultraviolet therapy, and debridement. Supervised by a physician specializing in advanced wound care, all consultations are conducted onsite and care is supported by our own certified wound care nurses.
Memory Care
Avantara Lincoln Park offers a holistic approach to quality care and services for those with Alzheimer’s and Dementia. At Avantara, we treat each individual and recognize their physical, mental and social capacities and/or limitations.
Orthopedic Rehabilitation
At Avantara Lincoln Park, our primary goal is to help our guests recover their strength and restore their confidence and independence. Our comprehensive and therapeutic approach to post-hospital rehabilitation begins with an intensive physical and occupational rehabilitation plan designed to meet the specific needs of each individual guest. Each plan is designed to help our guests reduce pain, improve living skills and enjoy greater overall functionality.
Short-Term Rehabilitation
Avantara Lincoln Park offers its guests one of the highest patient to therapist ratios, one of the largest state-of-the-art rehab gyms and an attentive level of personalized therapy seven days a week. On average, our intensive rehabilitation program can allow guests to return home in as little as three weeks.
Our adequate staff allows us to ensure person-centered care and helps us provide each guest with a highly individualized therapy regimen that can help shorten recovery times and facilitate a faster return to independent living. Along with restoring strength, independence and confidence, our comprehensive and therapeutic approach is specifically designed to help our guests reduce pain, improve living skills and enjoy greater overall functionality.
